Parker & Castle Rock Centers for Audiology accept most insurance plans. Please contact your insurance provider prior to your appointment to ensure that your visit will be covered and that all necessary referrals are in place. We require proof of insurance at the time of your appointment. Payment is expected at the time services are rendered.
Important Note About Your Insurance Benefits
Please be sure to check your insurance benefits. Treatments and procedures recommended to you are considered by your insurance company as a separate charge from the office visit with your referring physician, even if the appointment occurs on the same day as your appointment with your physician. Usually, co pays and deductibles apply to these types of appointments. For specific questions regarding your insurance policy guidelines, please contact your insurance provider's customer service department.
Referral Requirements
If your insurance plan requires a referral, you will need to request the referral from your primary care physician prior to your appointment.
Private Pay Arrangements
Out of pocket (or private) payment for hearing care services, hearing aids, and products not covered by your insurance plan are due at the time of your visit.
